Here you will find all the information needed for your appointment.
What to Expect For Your First Visit
Please arrive at least 15 minutes prior to your scheduled appointment time.
You will need:
- Drivers License or Valid Photo ID
- Insurance Cards
- Office Co-Pay
- List of Current Medications
- Any Reports or Imaging CD’s (unless completed at North Cypress Medical Center)
You will be asked to fill out comprehensive medical history forms. Please keep all medical forms with you until a medical staff member escorts you to an exam room.
If you are a new breast patient be prepared for an in-office ultrasound and breast exam at the time of the visit.
If you arrive late to your appointment, wait times may apply.
